Documents can be uploaded directly to your account by following the steps below:
- Log into your Clarity Pediatrics account
- Select ‘Healthie’ from the menu
- Log into Healthie (if you are unsure of your log in details, select ‘forgot password’ and reset your password
- Connect to your child’s Heathie account using the ‘Switch’ feature at the top of the page
- Select ‘Documents’ from the menu
- Click Upload Document
As a reminder all diagnosis documentation and assessment forms must be completed and uploaded at least 48 hours before your appointment.
