A series of standard medical, insurance, and consent forms must be completed. You may also be asked to provide a copy of your child or teen's ADHD diagnosis from a healthcare provider. These forms usually take no more than one hour to complete.
In the event that you are missing any required documentation before your first appointment, our team will reach out via email with a reminder to share or upload it. Please be aware that our Clinical team needs all the documentation in place at least 48 hours before your appointment
As part of our ADHD Screening for children ages 5 - 12, the child who is being evaluated for ADHD must attend the first 15 minutes of the second screening appointment alongside their parent or guardian. Additionally, the child's teacher must complete a teacher-specific form ahead of time. If the child is unable to attend or if any of the parent or teacher forms are incomplete, the screening appointment(s) will need to be rescheduled.
Your child does not need to be present for a Program Consultation appointment.
As part of our Intake for teens ages 13 - 17, the teen who is being evaluated for ADHD must attend both appointments. Additionally, a teacher must complete a teacher-specific form about your teen's behavior at school.
